Does anyone know of a mod for the AC4TV that allows you to run the internal speaker along with the internal one? I know BillM offers this for the Blues Jr. I have.

just a simple rewiring of the existing jack so the internal speaker doesn't get disconnected
very simple, have a look and you'll see

be careful doing that. adding a speaker will alter the ohms and you don't want to blow your amp by pushing too much of a load.

i've stopped using the attenuator and that solves the ohm issues,
the output transformer of the Ac4tv is large enough to handle a 8ohm speaker, if you don't use the attenuator, if you want to be truly safe then you'll need to change a resistor/cap on the board then you can again safely use the attenuator,
i've been using a 8ohm speaker for a long time now without any issues at all, but like my advice i do not use the attenuator at all ! !
